This document is used to make application for a new certificate of title, to replace one that has been lost or destroyed.

Submitting this document
Applying for a replacement title can be a simple process, but in some circumstances it can very complex. Read the SRO guide (see under "DOWNLOADS" below) and determine whether or not legal representation is required.
